A medium-sized pattern -- not mosaic size and not big 12-15 inch blocks. Generally something in the 6-8 inch range, either ceramic tile or patterned vinyl.
Small bathrooms tend to be dark, so a light color helps brighten things up.
Breaking the tile pattern helps too, it keeps your mind from measureing out the space based on block sizes. Here is what I did in my 5'x7.5' Mini Master bath:

I don't know whether to love it or hate it, but it certainly is interesting. How'd you handle the transistion with the dotted stuff? Is it really one layer of tile over another?
Well, tile OVER tile is a recipie for disaster!
I saw something the Bryon tile guy did awhile back that kind of inspired me on this. I wanted organic lines mixed in.
I accomplished it by making color photocopies of the different tiles (1"=1" scale), then laying them out in diferent ways until I got the desired effect. My theme for the bathroom was "where the sand meets the sea". I then cut the paper shapes and transfered them to the tile pieces by tracing it with a Sharpie.
I took those tiles to a little 5" tile saw, and initially notched the cuts in the vertical plane, knocked the waste off, then finished the cuts by running each tile horizontally across the leading edge of the blade to do the tight contour work.
Pretty cool, I say.
Light colored tiles and paint on the walls make bathroom look much bigger in size than it actually is. Moreover, you can also get mirrors fitted on one or more walls and make it large.
Most people don't want to see that much of themselves while in the bathroom.
Speak for yourself, it's everything I can do to not kiss my reflection every time I walk by.
