I am replacing old carriage-style doors on my house; does anyone know of a good manufacturer that makes a roll top that looks similar to an older door? I have checked Clopay and some others, but haven’t found any pricing info.
Is it cheaper to buy a flat paneled door and add moulding to trim out? If so, how do I add windows?

I have built doors like you mentioned( see attachments) from scratch and have bought flat panel doors and added wood to create the look.
The radius door I made myself and the other one is just cedar laid over the flat panels, I'm only guessing here, but I would think that you could cut windows into the flat panel doors, I'd have to look them over before guaranteeing that.
I think I can completely make the door cheaper than I can buy the flat panels and add the wood to create the look.

If you do make your own doors which I looked into for my three, be aware of the weight when you buy your track.
