Crushed by a "traffic plate" just gotta suck.
On our 20 story condo we have been complaining about overhead loads.
Our biggest complaint has been the "cradle" lift system verses the "choke" method.
The GC's response was----- "It's a construction site & by nature is inherently dangerous".
That attitude has lead to several calls to L&I.
i did not think chain was legal for lifting, I thought it was cable chokers or straps

I have heard crane operators say that the Q is not IF you will have an accident but WHEN.
I had a very close friend die as result of a similar trackhoe accident. He was a very experienced operator, trying to clear a small log jam out of a culvert that was resulting in a flooded road. Machine tipped as he was reaching and lifting a load. Apparently because the flood had softened the soil and he went over into the flood waters. He drowned, unable to get the door to the cab opened.
