What is the standard grade to follow for a driveway? The front of my garage is 3′-6″ higher than the curb height, so how far do I need to be back to have a good grade for kids playing in driveway and so on.

This probably will have a lot to do with how far the garage is from that curb. about a ¼ mile will give your kids a nice area. Joe H
I'd venture to guess a slope of 12 to one would work well - That's 12" horiontal for every inch of rise. That's the slope that handicap ramps are placed at.
But that would require a drive that's 42' long.
Is this a house that's built, or in the planning stages, or what? Are you somewhere that gets ice and snow in the winter?

The way that I have it drawn out is about 40' from curb. I'm trying to adjust the back for a walkout without having to shape the ground much. From the front of my garage to the far back of house I have 6'-6" of fall. I plan on raising up the house 2' more so I can have a 8' basement. This will give me 3'-6" in that 40' of driveway. I will bring dirt around from back to the front and sides to shape land.

The way I heard it, 1:12 is the maximum permissible slope for a wheelchair ramp, and that a ramp of any length at that slope is actually too steep for a lot of people in unmotorized wheelchairs.
1 in 12 pitch is about 4.75 degrees. That is usually steep enough to be noted on the interstates as being steep.Jon Blakemore
If that's the acceptable wheelchair slope, I'd aim for a steeper rise than that from the curb in, so you can have more level area in front of the garage. I hate watching those balls roll onto the street everytime you miss a pass.
