Anyone know if there exists a lag bolt that has an allen head (or similar) rather than the standard hex heads? If so, who sells them? I need about size 1/4″ or 5/16″ and 2 1/2″ to 3″ long for an application where the bolt head will be recessed an inch or so where I cannot get a wrench or socket on it.

I've seen bolts of that sort on playground equipment.
Most lumber yards have Timberlocks and Torx-head structural screws that are sold as lag replacements. The Timberlock can be driven as deep as you'd like and have a hex head that is smaller than the built in washer. The Torx-head structural screws typically aren't as strong as timberlocks, but have a much larger head so are better in some situations.
How many you need?
If one or 2, have quickly made that type specialty with a hacksaw and welder.
