I’m installing 1/2 OSB over skip sheathing for a new roof. The distant from the gutter to the ridge is 16′ – 4″. Is it alright to install a 4″ piece of OSB at the top or should I cut back the last full sheet and use something larger at the top? The last 4″ sits right over the last skip sheeting if that makes a difference.

Don't figure on a ridge vent covering 4 inches. As I remember you only cut 1 3/4 inches from center each way for the ridge vent.
It sounds like that's an old enough house to have a ridge rafter. You might have to bevel the top of the ridge rafter to allow enough space for the ridge vent.
As far as the 4 inch strip at the top - it probably depends on where your skip sheathing "skips" at the top. If you get a good break there's no problem. If you don't you could probably just adjust your top 4 foot course to break on the last 1 x sheathing strip.

Thanks for the info. The break in the sheathing does fall over the last 1x4 skip sheathing. I pulled all the old nails in that last board and the OSB lines up just fine.
