Small bathroom needs proportionally small fan. Big challenge.
Problem: a 12″x12″ or 15″x15″ fan (grill face) in a 9’x5′ becomes an eyesore. It’s complicated by a flat roof with 2×6 joists.
Fantech has a perfect small fan/light combination in the PB series (pic attached) – the unit could be aligned with existing lights make it less conspicuous. However, the fan will not fit in the joists.
A linear slot grill like one Expella sells might work but that will put an inaccessible inline fan in the ceiling. And I don’t believe it’s distributed in the US (they Australian based). Maybe a linear slot with exterior installed fan??

The only option I can think of is an exterior wall or roof mounted extraction fan ducted to the grill of your choice. Search for "ERV grill". There are quite a few smaller grills available.
I like the ERV idea. I think you are spot on - a roof-mounted fan is the route to go. I even found Fantech offers one.
I also found a couple USA based linear slot diffusers which are primarily for commercial projects but they might have something I can work with. (Titus-hvac.com, Price Industries, Krueger-hvac.com, AirFactors.com)
