My current plan is to cut a slot on two opposing sides of the plywood box, slip in some plywood under the cap, and break up the cap with chisel and hammer and toss it from the roof (avoiding the dog if possible). This approach seems logical, but I thought that I would ask for any better suggestions (after all I can’t do this until I get my $#@% taxes done).

I'd give whacking the concrete cap with a short handled sledge a try; it may crack easily into manageable pieces. If it's not to far from the edge of the roof and you've got enough slope, you can make a slide/chute of OSB or plastic pit liner and let gravity take it off the roof for you.
