No such thing as fail safe but you can sure harden the target. Look up FEMA recommends.
Best to buy a safe room already built. My parents moved into a slab on grade home in tornado alley. We thought of building a safe room in the garage, but found cheaper and easier to buy it already made. The big issue for us was when we considered the door to the safe room. You need a door that can potentially push tons and tons of debris as your house could possibly all fall in front of the door. Need to be able to get out!
That,s my fear if I used crawlspace----I actually hardened up room in garage with the boiler so I could cut water and gas plus have an electric chain saw in there.
I understand i could use 3/4" on walls and real plywood on roof as both were a higher level of protection. then for safe room use 2 3/4" plywood together would give a high level of breakthru from debris.
