Short version is that you must keep a 60" turning circle and a 30" wide approach to the seat, add two grab bars at specified height ( 36 - 42" ?) and approachable lavatory sink.

Keep in mind there is no code enforcement for single family homes. Besides the bathroom, the entrance, kitchen, switches, outlets and operating mechanisms are also addressed in accessibility codes.
