I live in Vancouver, Canada. I'm not sure if the manufacturer is local or not. Go to a good yard or Home Depot and buy it in the floor tile area.Quality repairs for your home.

Personally. I'd lay down some 5/4" sleepers and foam insul between them then 3/4" T&G plywood if you have the headroom

Depending on the size of your floor, you're going to have to buy a LOT of self-leveling compound. I second the idea of sleepers with ply over them.
