Here is what happens when there is insufficent thinset under a floor tile, and a heavy load is rolled across. 18″ porcelain tile on a concrete slab, moving in a small safe on a pallet jack. Maybe 5000 pounds at the most. Yes, it might have been prevented with a layer of plywood, but that should not have been necessary.

I certainly would not have expected a tile on concrete to break either.
Of course that void underneath it did not help.
JimNever underestimate the value of a sharp pencil or good light.
Many of the larger format porcelain tiles have a concave back. You have to use a larger trowell and back butter them.
Replacing the tile will be easy. Matching the grout will be a challenge unless the leftover was saved.

Anybody who rolls a safe on a pallet jack over a tile floor without protection should expect to regret it!!!!!
I used to do the final inspections for a big shopping center developer and they were a little picky about tile setting in the main areas. We were instructed to use a golf ball and walk a grid pattern while bouncing it. Man, can you find the voids easy that way! Instead of the distinctive sharp DINK you got a dull DOWK. Never failed.
From your pix it looks like there wasn't a full mud back. The concrete looks spaled as well. Good luck with that.

Porcelain tile has to be set on a solid bed, this means buttering the back of tile as well as a good bed on the sub floor. Its cheaper to use a couple more bags of tile adhesive than to have tiles popping or cracking and then getting a call back from the client.
I broke a tile by kneeling on it! Guy I work with that laid the tile was unhappy with me, but I figured it would have broken eventually--it had a void under it--and it was better me than the homeowner.
