Does anyone have experience with electric pad heat under tile,wood, or laminate floors. Specifically, initial cost,efficiency, cost to run, and reliability. Is there a large difference in a 110v or 220v system? All information is appreciated.

Efficiency will be 100%. As with any electric resistance heat. (Not necessarily the lowest cost per BTU - usually not - but the highest efficiency.
Cost to run will your heat load divided by 3413 BTU per kWh. Then multiplied by your cost per kWh ($0.05 to $0.25/kWh depending on location).
It will be the most impressive under tile floors - quick to respond and a greater BTU/square foot is possible. It can be really nice in a bathroom to have warm feet. And it is a limited area and limited time of use so the cost to install and operate are minimized.
