I’m looking for a supplier of exterior MDF. Does anyone make it in 1x sizes (1x6x16, 1x8x16, etc…) and does anyone make it in 5/4 sizes (5/4x6x16, 5/4x8x16, etc…) How does it work for exterior trim? Should it be back and end primed as wood should?

Eric, the product you're looking for is GP Prime. Made by Georgia Pacific, it comes in all the sizes you ask about and more. It looks alot like the old Masonite hard board siding, but holds up alot better. We've used it on quite a few projects with good results. We scuffed some up with a utility knife and soaked it in water for two weeks, it looked the same when we took it out as when we put it in.
I'm sure your local lumber yard sells it.

I have been looking for the same product myself, never been able to locate any. Will ask for the GP stuff that Steve mentioned. (Easy for me to remember.)
In the July FHB, p. 109, is an article on using ext. MDF as surrounds for vernada columns. Article mentions sealing the MDF on both inside & outside, and holding the ends just short of the deck to prevent trapping moisture. GP.
*If you do a search of the archives for "prime trim" you should come back with some more info. The topic has been discussed before. FWIW I have used it quite a bit and it is a good product. Prime all surfaces just like you would any other material and it will probably last your lifetime.
