If none of the concrete gurus are going to weigh in on your question, here's my two cents: concrete slabs should be insulated with extruded polystyrene (XPS), commonly seen as pink or blue boards by Dow-corning et al. There are varying densities; make sure you use a higher one, either 15 or 25 psi (not sure when to use which). The perimeter of a slab is probably the most critical area; it will lose heat to the outside most readily. The slab / footer / wall juncture should be carefully analyzed. In my mind, insulated concrete forms would be the only way to build in your climate. See ICFWEB.COM for more info.
