Your standard GDO and microwave opener are cheap enough and fungible enough that specific surge protection for them generally isn't required. If you're in an area/situation where lightning-induced surges on the power lines are likely then you're probably better off spending your money on a "whole house" surge protector.
Individual surge protectors generally should be installed on high-value items (big screen TVs) and hard-to-replace items (computers) if surges are likely in your area. Also, any connection between phone or cable TV lines and a high-value/hard-to-replace item should have protection. (One spot that can be overlooked is a cable TV modem connected to a home computer network. This can introduce surges onto the home network unless well-protected.)
And, of course, any outside antenna (or antenna in an attic) should feed through an appropriate lightning arrester.
