I have a 5/8″ plywood subfloor that is out of level by 1″ over 12′. I am going to install 3/4″ maple naildown. What is the best way to level this floor?

Remove subfloor, shim joists, apply new 3/4" t&g plywood subfloor.

There's a couple of answers to your problem. First, 5/8 subfloor is the absolute minimum. 3/4" would be better.
Is the floor unlevel but flat? Or are there dips in the floor? Big difference. Imagine a sheet of glass ... you can tilt it so that it's un-level, but it will still be flat. If your floor is flat, then you don't have to dso anything to make it level before you install the wood. And if you do choose to make it level, will you be creating a problem with cabinets or door thresholds?
