I apparently installed the swag hook incorrectly as there is a significant gap between hook and ceiling. What is best way to remedy this? I assume me not having the toggle wing low enough (closer to hook was my problem.
How to I make this look better? Can I take another toggle/screw/hook and put in same hole, assuming there is room for two wings in there?

So is that a conventional toggle bolt, with two wings that swing out once pushed inside the ceiling? And did the wings not swing out, or did they rather swing out but leave the bolt hanging like that?
If the latter, the solution is simple: Twist the hook clockwise and screw it upward (while maintaining a slight downward pressure so that the wings "bite" into the ceiling).
If you were not able to get both wings to swing out fully then the opposite solution may apply -- unscrew the hook slightly so that the wings have more clearance to pop out (once the hook is pushed upward again). Then, of course, tighten the hook.
Im pretty positive the wings swung open. I tried both your suggestions, but wings arent biting onto anything. seems like an open airy space above the ceiling. I think I had the wing placed too high on the screw when I inserted it, and not low enough toward the hook. I think I need a shorter screw or just lower wing on the next go-around. I assume I can do a redo in the same hole as it seems pretty "airy" above the ceiling. wdyt?
You pull down on the bolt so that the wings bite into the top of the ceiling. The thing you might have bolloxed is setting the wings so high on the bolt that when they open they snap over the top of the bolt, so it can't easily get through. If this is the case you can probably tighten the bolt as much as possible, then wiggle it around to cause the wings to "flutter" a bit, then tighten some more.
Worst case you abandon the wings by unscrewing all the way and then buy a new set (on a hardware store toggle bolt with the same thread as your hook.
