I am looking for some ideas about a good way to fabricate a ceramic tiled top for a picnic table that’s left outside on a deck.
One idea was to make a form and pour a concrete table top to glue the tiles to but what’s a god way to attach the legs and /or supports?

You said "pinic table that is left outside on a deck"... Is this a wooden deck, or a concrete/brick patio?
If your "deck" was a floor grade patio, then I would say just form and pour a set of concrete legs as well. The "legs" could simply be a rectangular or square shaped slab at each end ; whereby the top rests atop these 2 slabs.
But, an all concrete table would definately be a weight problem for a wooden deck, sooo if this is the case, build your wooden picnic table out of the outdoor wood of your choice ( cedar, redwood, mahogany, Ipe, cypress, treated...etc). Then attach a sheet of 1/2 inch cement backer board (CBU) such as "wonderboard" or some other brand to your table top and then tile it accordingly. This would cut down on the weight tremendously and should give you the results you are after.

If you are set on wanting to mount a solid concrete top that you formed and poured to a set of wooden legs....you could insert long bolts in the wet pour. The bolts should be positioned where you want the top to connect to your legs.
Drill holes in the top of your legs, fill it with epoxy, then set the table top in place. The epoxy is waterproof and will make a strong joint connection. Just make sure your legs are stout enough to bear the weight of the table top ( ie...4X4 legs or 6X6 legs, properly braced)
