How much undercut/ fill dirt?
How much plumbing?
How much concrete per yard?
How much rebar?
labor rate? I say for the information you gave $ 19 437.81 but I could be off a couple pennies.

but a mono has a turn down footing which is usually a foot wide by foot deep here in the sunbelt. thats another five yards so rough guess 25 yards. 384 feet of rebar. Form rental or wood. wood 128 feet of 2x16. 64 3 feet 2x4 for stake, then strong backs. a 2x running $3 here so roughly 38 cents a foot for 2x4 600 feet( $228) a 2x16 10 feet $9 cheapy ($128) concrete $78 a yard, 25 yards ($ 1950) dirts &75 a truck load, about 11 loads. ($825)Look lets do it the easy way. My house is 28 x 40. concrete was 35 a yard my mono slab everything was a hair under $5000. three years ago.

$35 a yard!!!! That's incredible! I can never remember it being less than $60 in the few areas that I have lived. Ok - maybe I remember $57. Generally, I don't think of there being a huge difference in materials prices across the US as long as it's is not in a remote area. I guess concrete is more just raw materials, which can (apparently) vary widely. Right now, here in NC, plain 3000 PSI concrete, I think is $77/yd. It was $71 but I had a letter a week ago with a price increase. Do you know what the current price is where you live?

it running about $75 a yard right now, and the cement plant is 300 feet behind the concrete plant.
if you Google a bit you can find concrete calculators online....
40ft X 24ft slab @ 6 inches = approx18 yds concrete
add for thickened slab edge like Brown bag said = 128 LF x 12 inch width X 12 inch depth = approx 4.7 cu yds concrete
approx total concrete needed =23 yds @ 73 per ( Pittsburgh, PA price) = $1,679
if installing rebar mat on 1ft centers (highly recomended) then you would need approx 90 pieces of 1/2 inch thick rebar, each piece 20 ft long.
90 pcs rebar @ $5 per ( Pittsb. price) = $450
TOTAL COMBINED MATERIAL COST of Concrete & rebar only = $2129.00
In addition, you have to factor in site prep. forming, and labor...sooo total complete price would range between $4,500 and 5,000... if you hired someone to do this job.
If you and your buddies did the work free gratis, you could do the entire job "in house" for say $2,500 to $ 2,700. ( add a hundred more for the beer you guys will drink while waiting for the mud to harden enough so you can jump out on it with your knee boards. ha! ha!)
