We are working with a GC to build our stick-built, 2600 sq ft, single level home over a conditioned (ventless) crawlspace and need a metal or composite crawlspace door, approximately 36″ square.
North Carolina Building Code stipulates that the door be a minimum R2 rating.
Any suggestions?

I have similar crawlspace. I made my door out of plywood and 1x4's (stained with exterior eurethane) with 1.5 in insulation board on the inside [pressure treated 2x6 jamb and Sill]. Passed inspection in Caswell County, NC. You could try cutting a store bought steel door if you need steel, or putting a sheetmetal skin on a door that you make/insulate.

My mason installed a premade steel door. The frame had "wings" on the side and he just slid it wings into the side grooves in the CMU's that framed the opening.
It couldn't work better.

On a taller door, or a sealed crawl, we cut down a fiberglass slab, and site build a weather stripped frame w/ a therma tru type sill.
Lowe's contractor yard (since you're in NC, I'm assumig there's one near by) can get pvc doors in a pvc frame that pass code.
Rigid insulation on a weather stripped pt ply door also passes.
