I was watching the DIY channel last night, and noticed what I thought were mistakes. They were doing a bathroom remodel. They had put up paper faced insulation, then they put a plastic vapor barrier over that. After that they put concrete backer board on it. I always thought it was againest code to double up on the vapor barrier, am I wrong? This was in the shower/ tub area.

From a practical standpoint the paper facing on FG isn't much of a VB, and if you put up two VBs back-to-back (vs with insulation in-between) you won't have the "double VB" problem anyway.
The bad think is to have a VB on the cold side of the insulation.
