Can a 90 degree combination wye be used horizontally. Meaning all parts of the wye connecting to horizontal pipes. I need to come thru the foundationhorizontally and turn 90 to tie into sewr line outside and would like to come off the back end turn 90 vertically and extend pipe above grade for cleanout. Thanks guys

I wouldn't use a wye for a cleanout riser. It is directional, either upstream or down stream. Depending on which direction a blockage may occur you are limited to the direction a snake can run by the direction of the wye.
Come through the wall, then turn your ninety and follow it with a non directional tee for the cleanout riser. If you ever get a blockage on either side of the tee, you can send the cleanout cable either way.
But it can be used horizontally(laying on its side) I will try to explain better. The curved 90 part will turn and connect to sewer outside going downhill. The straight part will get the cleanout plug or i will extend above grade. This is only to access straight run along house till it hits cleanout sticking up in front yard.
The wye will only have a 2' piece running thru foundation to another combo wye. This wye will turn up to my main stack, and there will be a cleanout there. I figure this way tha part i consider most vulnerable to blockingIMO will have access to both directions.

If I understand your plan, it seems fine.
You already have an inside cleanout that could be used to snake the line thru the foundation and on past to the next cleanout in the yard,and, in addition, you will install a new cleanout just outside the foundation.
All connections will be by means of wye fittings or 45's, and this is what the code prescribes for horizontal to horizontal connections.

Unless I misunderstand your proposed configuration, I'm not happy with your plan.
You can use a combo fitting on its side (horizontal plane). But you should do that with the curved connection used only as a DRAIN (not vent) INLET, not as an outlet.

Jim, you lost me on the used only as an inlet not as outlet.Isn't it an inlet on one side and an outlet on the other? :) Do you mean it should only be tying another line into a main branch.
The flow comes down vertically,sweeps thru a combo wye(standing on its flat part)travels20" thru a horizontal pipe then sweeps thru a horizontal combo and connects to the pipe running along the house(20'+) that connects to city pipe.The back leg of the horizontal combo is a cleanout.It is in line with the pipe running along the house. I hope maybe that is more clear.
May i take some more of your time for another Q while i have your attn. Can a Sink drain line be in an exterior wall? I would rather run it up thru the floor but that makes venting hard.
A combo fitting has 3 connections. 2 are always inlets; 1 is an outlet.
An inlet is used to accept wastewater into the fitting or for a cleanout.
The outlet is used to let the wastewater flow out of the fitting and on downstream.
With a combo, the "curved" connection and the straight connection closest to it are always the inlets. The straight connection off by itself is the outlet.

It is done all the time. Drain lines as subject to freezing because they should never have any standing water in them. Once clooged, however, all bets are off :)

Studor vents are not true vents, you never use them if you can run a real vent!

If you do this you are using the combine backwards. If you are worrying about dual direction clean-out access put in a test tee.

I have two test tees, sorry it just sounded funny to me. I am pretty sure i have the flow on the combo right, I know which way you mean ,I think.
