I would like to install 3/4″ hardwood flooring in my family room. It is on a concrete slab. I was advised to put down a 3/4″ plywood subfloor and attach it to the concrete with adhesive and a powder actuated nailer. Is there a preferred adhesive? What length nail should be used and what should the spacing be? Should I be concerned about the lack of a moister barrier? The increased height of the floor is not a factor, I am installing a new door. The house is about forty years old.

Contact the MFG. of the subfloor product for their fastening requirements/nailing schedule. They should also be able to tell you if they recommend a vapor barrier.
If you can't get any info. from the mfg. I would put down a 6 mil vapor barrier, and consider using Advantec for the subfloor. Space your sheets to allow for expansion.
I would using Grip-Cons for fastening the subfloor. If you are not familiar with them you predrill a hole, then nail. This assures every nail holds tight and prevents the spalling (creating a bump in your floor) and loose nails that are common with powder driven nails.
