I have a client who wishes to convert an attached garage into a den/office. He would like me to frame a 2X6 subfloor over the existing slab. The house sits on a crawl space that is not insulated. The floor of the house is not insulated either. This crawl space is accessible from the garage through a hole in the slab. The garage slab slopes from back to front 2 1/2 – 3 inches. I will need to fur garage slab up at one end to level new floor, this will create a small space between slab and new floor joist. Should I lay a good poly vabor-barrier on garage slab and add vents at exterior walls of garage and treat this like the crawl space on the rest of the house? Client would like new subfloor insulated. I am concerned this will restrict any air circulation since new subfloor is so close to slab. I do not know if the slab has a vapor barrier under it. This house is located in Northwest Ohio, any advice would be appreciated.

Sounds like there might not be enough clearance for a crawlspace. (12" under structure) and certainly that is tough to crawl in or insulate.
Consider a raised slab on sand fill with either quality underlay for softness or 2x4 strapping and ply subfloor with 1.5" styro insul.
Either way, there is no garantee the original slab had poly under so include it (as close to the warm side as possible.)
ps. If you decide on framing, remember to allow for shrinkage. Maybe not so important in a doorway but very obvious if you are removing walls and tieing in to existing. Go for KD if possible.
