I need a heavy duty hinge that pulls the door forward before it turns 90 degrees. Please help.

There are cabinet hinges like this, but any "heavy duty" hinge with the property would likely be a specialized producter.

What pulls forward and then turns 90? Most hinges pull out away on their way to 90.

so here is why the pull out before 90 degrees
The door/panel is 3 to 4 inches thick. Its made outta foam and plywood I need it to clear the thickness to be able to turn to 90 degrees.
Round over the inside frame of the door
Assuming your insulated, 3-4" thick door has a wood frame, just round over the inside edge of the hinge-side stile.
Another possiblity. Cut and assemble your door frame as is commonly seen on an entry door. The frame is rabbeted and in addition there is a curf that holds weatherstripping. Done correctly, there should be a 1/4+ inch gap between the inside face of the door and the frame all around. Now install conventional weatherstripping.

Doors will be beveled or back cut because of the bind you mention. However, in your case or with doors in a castle, a rabbit or some other method to provide a nominal air seal would be the norm. I’m not saying that a hinge like you’re looking for doesn’t exist but I’ve not heard of them

Now I understand what you want. Calvin is correct, in 50 years of carpentry I've never seen a hinge like that. More typically for a door that size would be face mounted strap hinges and the lock edge of the door would be rabbeted to standard door thickness so the edge will clear.
I have seen arrangements with a doubled-up hinges on a flap, allowing the door to be first pushed more or less straigh out, before "swinging". Complicated and awkward, but it seemed to work.
