Home with no vapor barrier beneath concrete

I live in a new construction home with slab on grade. We have water coming up through the concrete and seeping through the vinyl plank flooring. We removed the flooring and discovered a wet slab throughout the house. When the concrete in uncovered, it dries and appears to stay dry. When we tape plastic to the slab or place a piece of flooring on the slab, water comes up through the slab and is visible within a couple of hours. We are investigating if a water vapor barrier was placed on the ground before pouring the slab. If not, and if this is the problem, any ideas on how to remediate? Or if we have the proper barrier, any ideas on how to remediate? We applied a deep penetrating sealer on top of the concrete, but after two coats it seems to not help. Is is possible for a concrete slab to be bad, not caused by water under the house, but rather not prepared, cured, or handled correctly?

You either have a very high water table or you are built on a spring. I would dig a deep post hole to try to find out. A deep french drain around the perimeter that drains to daylight should relieve the pressure. If this is new construction where is the builder? Should be his problem to solve.
