How does one build a shop slab founda…

I am planning on building a new wood shop and metal shop this summer. How does one prepare to build a slab foundation? I need a heavy concrete floor to support heavy machine tools. I also live in colorado where frost heaving may be a problem. What would be the best design for a simple concrete slab/foudation for this shop? Thanks.
