How to correctly hang a modern ceiling light fixture from a Victorian-era junction box?
See the photos – how best to correct this situation and have the base of the light fixture be close enough to the plasterwork that no ugly gap is visible from below?

There may actually be a box up there, above the cast (bronze?) cover plate, in which case you could mount it flush, sort of. Or this may be old K&T wiring where the wires come directly from the K&T run, in which case probably your best bet is to remove the plate, install an octagon box in its place, and mount the fixture to that.
Possibly some sort of thin sheet plastic could be formed around the base in a way as to partially hide the difference in circumference.
Use a bell canopy in there up side down so the small end is at the top and big end mates with the pancake canopy you have. Cut the small end hole to match the top hardware and pull it up until it is tight.
I would start by looking at ceiling fan canopies They are close based on the way I see your picture
