I’ve got the old v18 set(1 1/2 yrs). I’ve tried to check a couple of times to see if I could use the new lithium-ion batteries – no luck.
Question; can I and is it worth it?

Yes you can use them. They fit anything that is 5 years or younger. Lithium Ion doesn't give you any more power, it's just that the batteries are lighter and last longer to a charge, and I think may have a longer life span. There are some draw backs however, they are almost twice the price, and they don't charge well at all in the cold so I'm told.
I too have Milwaukee 18v cordless tools and I'm on the second set for my drill, and got the regular NiCa set which works just fine. Last weekend I picked up a Sawzall and Circular saw on sale without the bats, so now I just ordered 2 more 18 v batteries. Get them on E-bay, fresh from the factory for half the price they are in the store.
