I want to put in a mirror and have romex hangin off the drywall. What is the proper way to put a box in for lighting? This would be a mirror with cut outs for lighting.

If you put in a box, the hole in the mirror needs to be large enough to allow the box to be completely accessible.
There are some lighting fixtures that do not require a box, but can be surface mounted and fed the romex directly, serving as their own box. But it would get ticklish with one of these as the romex would have to be protected somehow from the edge of the mirror, and an inspector might have "issues" with the fact that the romex would have to be disconnected from the fixture if the mirror needed to be removed.
Are there "remodel" boxes that go in from the front? Not quite sure how most folks do this.http://www.etherhuffer.typepad.com
Yes there are old work boxes that could be used. But you need to check the range of the clamp and make sure that that it can clamp both the drywall and the mirror thickness.If I am not mistaken typically the boxes are set and then the glass company makes a template and cut the glass to fit including the hole fo r the light fixture..

I would talk with the glass company first and make sure that you are both on the same page (or the same side of the mirror in this case). But yes..

Typically, as has been said, place your boxes dead on, glass supplier will template. A word of caution, placed boxes, 4" octogon, mirror went in, fixtures show up and their made to be mounted on a 2''x3" switch box. A little dancing around to get that to work out. So, check the fixture first.
And be aware that, if things are too close, the hardware will show through the glass at the edge of the opening.I frankly would say "fork it!" and cut the mirror in sections, putting the lights between. Save a lot of grief.

Oh yes, Dan, I've suggested that many times, so much easier, but then the freakin' architect or dizzy designer get in there, "but it just has to look like this, nooo, we just can't have a seam!!" Drive me crazy
