A mason is pouring raised patio for fill he used a slab he broke apart and placed it into the three foot high block wall. Will this cause a settlement problem in the future? there is about 20″ of old concrete with know other fill.

Quite possibly, unless it was well compacted, and that doesn't seem possible with chunks of old concrete.
Do it right, or do it twice.
Unless there is a way of filling the voids with compacted fines, there will be settlement.

What about filling with sand and compacting with a garden hose? Might take quite a bit of sand and some time, and hoperfully the excess water would drain out, but ...might it work?Do it right, or do it twice.
