First anytime you run plastic pipe in a slab it should be protected.
Rubitex¯ & armaflex¯ are good choices in a pinch foam rolls & bubble wrap work too.
Not sure what you mean by a "wye" or a sweep at the wc.
Generaly you have a short turn 90 under the wc then run to closest wall then with a "combination" on it's back to run off for the vent.
You should talk to your building inspector about running the pipe thru the concrete. Here they do not allow the pipe horizontally thru the footing. It can go under as long as the footing is at least the minimum depth per plans, and it can go thru the stemwall. If you have a toilet backing up to an exterior wall in a slab-on-grade house you may have to run the vent thru the concrete to get into the wall.
Anyway, talk to the inspector, because when he comes to see your forms he's going to look at how you've handled the pipe.
Same here.
We have to go under the footing or sleave through the foundation wall. All underground pipe has to bedded in and covered by crushed stone.

Don't know about your specific question, but be sure to get your inspector in early and often because they aren't too understanding about you covering up work they haven't approved...
