How high above the roof line is your stack? Many modern building codes require a great deal of stack so it will draft well, but outside of inspections many are built to look good, not function properly.
If you are in a windy location the relative pressure outside at the stack and inside the house will be different. Enough of a difference in the wrong direction and you'll always fight it when the wind blows. In good ol' windy Wyoming it was common to find new poorly thought out fireplaces that couldn't be used on windy days.
