I am looking for cover plates that have a type “f” connector for video, rj-45 for ethernet and a rj-11 for telephone all on one coverplate. Can anyone tell me where I can get these? Troy

Try Newark Electronics. This is a catalog supplier of everything electronic you could ever imagine (and some you probably can't) They've got 1,500 pages of meters, coverplates, connectors, cables, power supplies, etc. If it exists, I bet they'll have it.
