I have a kitchen island I would like to put a tile top on .What kind of build up do I need under the tile one side has a 12″ over hang. Do I use ply wood and cement board.

Even with 1" plywood and 1/2" backer board, 9" or 10" inches is where I draw the line for overhangs. Bitter experience speaking here. You need korbels or, if that will be too clunky, well attached cantilevered steel, every 2' or so.
A big guy leaning on that overhang will crack grout, or worse, tile.
