I’m building a home with a wine cellar that will be below the grade of the finished basement. I want the cellar door to be hidden and imagined I could buy a “trap door” that is flush mounted with the basment floor and opens to the cellar stairs. I have looked everywhere for such a product with no success. Does anyone know where I can find such a door? I’ve looked at the wine cellar sites and while there are many doors to choose from, they are all conventional vertical-mount doors. I may be forced to build the door myself and think I can handle the construction and insulation of the door, but would need help with the hardware. Any ideas?

Might try looking at commercial fire hatches.. seems I remember some that might work for you .
Interesting application! I think you're going to have to build it yourself. You'll need the barrels of standard hinges flush with the finished floor, which would show. Another possibility may be some FOL Euro-style cabinet hinges made for thick doors. They are made for up to about 1-3/8" thick doors, fitting flush all the way around. Your floor and door would be thicker, but could have a 1-1/4" support lip and ledge all the way around.

Consider Soss hinges. They're invisible when the hatch is closed, and they open 180 degrees, so that the hatch can lay flat on the floor when it is open.
