Hypothetically, if a fluorescent fixture is wired to the ceiling junction box with the switched hot and neutral spliced in reverse to the ballast, would this cause the ballast to fail prematurely?
I had a a handyman install a fluorescent 4-tube fixture for my Father (in another state), and two separate fixtures have failed on this particular junction box in a short period of time. I have not had the chance to check the situation out myself, yet.

Depends on the ballast.
Assume it is a 120 Vac standard ballast, then no for most types.
Some electronic ballasts would fail, depending on the circuit type.
277Vac ballasts would fail within a few months every time, but your father would need to be on a 460Vac 3 phase system to have 277Vac.
