Greetings
I have been invited to build a 850 sq foot deck on a flat rubber roof four stories off grade. My first concern is safety: is there a way to make my staff of two safe during construction that others have used with success? The perimeter of the new deck is ten feet from the edge of the roof, but still that not reassuring. Some have suggested a harness, but where to tie in? Also it may create more hazards than it solves. thanks

There's gotta be a window or door, put in a 2x4 perpendicular to the opening and tie harness's around that.
Not sure if this is feasible, but you could attach 1/4 inch dia steel cable to house structure and run the cable out to the opposite end. You could attach a 1-1/2 inch X 1-1/2 inch steel angle iron to each outside corner of the building. Drill holes thru the angle iron and run the steel cable thru it.
The cable would encompass the entire perimeter. You could string cables at various heights. Tie off yellow caution tape vertically to the cable to act as streamers so to keep your men cautious about how far to wander. You could instead drill holes thru 1x lumber and string them thru the cable instead of yellow tape streamers.
The idea of this is to act as a precautionary border so as to keep your men aware of where they are and what they are doing. This cable is not really for tying off harnesses to..instead it permits you to work harness free. Not sure if this would be OSHA approved...probably not.

I have seen something like this in action the other day when there were some men making a repair to a roof. It seems the whole trick is to avoid accidents by being aware of where you are at all times. I include a photo herein of the work area and invite others for their input. With the thousands of roof decks that have been built, do I assume that people do it without incident with the aid of lines and warning flags?

I'm sorry to stick a pin in your bubble, Dan, but if you are worried about falling off a flat roof with a ten-foot buffer zone between the work area and the roof edge, maybe you shouldn't be considering doing this kind of job. Not everybody is comfortable working at height, and you're not required to be one of them. But if you can't handle that easy scenario without some kind of 'safety' gear, you might want to re-think the whole thing.
