Does anyone have a somewhat elegant way of mounting a ceiling fan (or hanging light fixture, for that matter) from the apex of a cathedral ceiling. One way I have thought of is to build a little box with top cut at the pitch of the ceiling in which I could mount a fan box. I’m concerned that it might ruin the clean look of the ceiling. Is there anything better?

I've seen the solution you propose and it's the best looking method I've come across. A variation is to bend the box from sheet metal. You get a flat, square bottom with triangular gables. That way, there is no visible thickness to the bottom and it doesn't stand out so much against the slope of the ceiling. It would be painted the same color as the ceiling.
Had good luck with these or similar.

Arlington Industries makes a lot of good products. They are my favorite push-in NM connectors. Strong enough to hold well but flexible enough to install without a fight.
This is how we have done it in the past (new construction).
