Our home was built in the mid 60’s and we need to replace the back door The only problem, I understand,is that we have a concrete threshold. Can a “standard” door be installed and still use the existing concrete threshold or must the new unit be installed with its own threshold on top of the concrete?

Depends. MY GFathers house in Wildwood NJ had a concrete threshold, because the back bay could flood during 'canes. The 4" high step helped keep seepage at bay.

You need an exterior door to seal all the way around from weather and air. Most exterior doors come all pre-hung in their jambs along with a threshold/sill. It should be installed as a complete unit.
