I've asked this a few times and i've only scored on one occasion. Email me and I'll reply with a few that one guy sent me.
I have started using the ones available in "The Contractor's Legal Kit," available from Hanley Wood through JLC's website.
I use that program also. Has worked great for me so far. DanT
Heres one we used this spring with a large volume builder.
Proposal and Acceptance
The Proposal
This Proposal, made on March 19, 2004, is made between:
Advanced Construction Concepts, L.L.C., of 36716 Melbourne, Sterling Heights, MI 48312, hereafter referred to as “The Subcontractorâ€,
And
Tad Homes hereafter referred to as “The Builderâ€.
The Subcontractor, agrees to: rough frame a 12 unit stacked townhouse (6 Liberty I’s and 6 Liberty II’s) in the Stonegate Pointe community, in Pontiac, MI and to supply all labor, nails and screws and all crane/forklift fees needed to erect the rough frame, for the sum of One Hundred and Three Thousand Dollars ($103,000.00). This agreement is subject to the terms and conditions listed below.
Terms and Conditions:
The payments (two draws) are to be made at the completion of each stage. The Builder agrees to pay to The subcontractor:
1) $50,000.00 at the halfway mark on April 7th, 2004.
2) $53,000.00 at the substantial completion of the rough framing operation on, or about, April 21 (weather permitting) and/or no later than 7 days after substantial completion.
Passing the Rough Frame inspection shall constitute the completion and fulfillment of this entire agreement. If the Subcontractor fails to pass the rough frame inspection, on the first inspection, then he agrees to pay a re-inspection fee of up to $100.00, if any are imposed by the local building authorities.
If payment is not received at the completion of each stage (as specified above), further progress may/will be delayed until said payment is received and the account will be considered “past due“. A 1% per month late fee will be imposed and will be added to all “past due†accounts.
Any changes to this agreement, or to the plans and specifications must be made in writing. Any additional work done as a result of said changes will be billed at the hourly rate of $58.50

I manage to get all that one one standard sized sheet. I won't vouch for the effectiveness nor the legality of it. I wrote that one myself. It is very vague, yet specific. I suppose if they didn't want to pay, there would be lots of areas to argue.
I write different types for different types of people/builders.
