Own experiences, if it makes sense to you, by that I mean if you forsee something being an issue, resolve it ahead of time and include it
In a general sense you can write a contract completely in plain English so it makes sense to you and everyone else and use that as a good starting point, hire an hour of a lawyers time and have them help you make it so it legally is worth a spit where you are
But my real advice is define it. Everything. The scope of work. Things you include. Things you dont. Specifications on anything you're providing (i.e. GE model 12345 oven, white), anything you're specifically excluding.
And then contingencies. You cant cover each one individually since they're virtually infinite but try to address things like unforseen conditions (see the book above) or if you're going solo and oh! Now we are going to need an electrician/plumber/whatever . . .
Edit: and if you want to read about all the reasons not to cost plus (which might just save you from the reasons themselves) search it here, or Stones Markup & Profit IIRC he addresses that almost a little too thoroughly.
