Cutting Roofing vents under tile roof

My roofer forgot to install the roofing vents to my cement tile roof. So when he came out to install them, he basically cut a square hole through the tile roof AND the roofing plywood, then installed the vent on the tiles. The plywood has a layer of asphalt sheet membrane layer adhered to the plywood. Now that the plywood is cut out, water can enter the house through that hole. Is this ok? The reason they put the roofing membrane is to protect it from water that does escape through the tiles. But if holes are cut through the plywood to allow the roof to vent, is there not supposed to be something placed on the uphill side of the hole so that water is directed away from the hole? What is supposed to be done in this case?

Yeah, that's not right. They need to be flashed in over the lower tile and under the upper tile.

Could you elaborate on this? I made a crude picture to show my real concern, which is not the roof vent attaching to the tiles as I think it is tucked underneath the upper tiles ok. My big concern is cutting of the roof deck plywood without some kind of protection preventing water coming through the cut out. Please see the red area of concern in the picture. Thx.
In the description of your waterproof paper you said it was adhered to the plywood.
Is it a paper or a flexible rubber like product? The flexible adhereing type product might be ice and water shield?
Either way i think it should be flashed. I'm not a roofer though.
It is called "ice and water shield." You think that they should flash that plywood above the cutout? Would make sense to me also as water would eventually make its way there and into the house.
If I were installing the vent and the tiles were on battens, I'd rim the opening in the sheathing with batten material, either caulking under the rim battens or bringing the i&ws over the top of the batten on the top of opening. It needs to be flashed. The ice & water shield is where your roof is really sealed against water. The tiles on top shed most of the water and keep the sun off of the I & W shield.
So not having it flashed kind of defeats it's purpose.
I'm not roofer though.
