We are installing 2 separate jetted tubs, the top of which is 20″ AFF. Is it typical to have a step up to ease entry into the tub? If so, what size/height?
Also, where should a grab bar be located?

Steps are common, but dangerous. Unless a really grippy surface material is used they can be slippery, and without something to grab onto nearby it is easy to loose one's balance, too. Lots of serious injuries have occured on such steps.
It is much safer to not have a step.
I agree WayneL5. I dont' like to step up onto a step, then over and down into the tub. It might be easier getting out, but getting in is more dangerous in my unscientific opinion.

The answer is generally "No," but I have a caveat.
The best way into these tubs, often, is sit on the edge and "swivel" in. Now, what that suggests is a very low step, in this case maybe 4" tall, to give a 16" seated height spot to enter/exit.
Naw, that does not need to be full-width step, either (which can get in the way of getting cloes enough to clean, too).
The step, as previously stated, has to have some grip to it, and not some of the time, or mostly, but all the time every time. But, that's just my opinion.
Now, for grab bars, those almost need fitting to suit. They should not be farther than arm's reach (about 12") from the point where the tub entry/exit is. Generally, they need to be deeper into the tub than "looks" right, too. Trouble with all that, is that then it looks like pool accessories.
