I think the NEC specifies that the fixtures be listed as "Suitable for Wet Locations"

The NEC says only that any man door (uh, personnel door) in a residential occupancy must have an adjacent exterior light fixture. No specification as to the height, or the horizontal distance from the door.
I set wall-mounted fixtures at 81" to the center of the box. Why 81"? That's the way my journeyman did it. And, it's high enough so that you're not staring right into the fixture, yet it can be re-lamped by most people w/a step stool or 4' ladder.
The distance from the door depends on whether I'm installing two, and the available space. If there's a window adjacent, I'll split the distance if it's two to four feet, or go 18" to two feet away if it's longer. Adjusting for studs and blocking, o' course.
But it's all up to you.
