I’m working on a design that is 4.5 floors. The half floor is a tower room. The house will have an elevator stopping on all floors. Square footage is tight on the upper two floors. If I have exterior stairs leading all the way to grade, will code require me to have interior stairs on all floors?
This is a single family residence on a steep hillside lot in Chattanooga, TN. Our inspectors use 99′ standard buliding code and 97′ Life safety Code. I work for a spec/semi custom builder and we have never used elevators before.

First you need to describe the application (single family, multiple family, commercial, mixed use) and what code you are taking about?
is the tower a turret? Can you enlarge it encasing the elevator?

Don't know the specific code, but I suspect you're required to have two means of egress from all floors.

Go to the source . . . your local permit official. An elevator is a costly piece of equipment. Even a 'LULA' (limited use/limited application) elevator can run between $40,000 and $60,000 (price quote from a supplier a couple of years ago).
