What is the difference between “general purpose” and “combination” table saw blades? I can see the tooth patterns are different and read that one has a raker tooth, but how do they differ practically?

Really there is no difference. Both terms are really just choice of descriptive words, not technical terms with any certain meaning.
Things like ATB, Raker, kerf size, angle or pitch and number of teeth all are combined to make up what sort of blade is best for a particular job. A combo or all purpose is a compromise of other details.
