I am adding a piece of mdf (1/2″) to an attic ladder door that is luan. size 21″x 51″.
I would like to glue it and back screw it until glue holds.
What type of glue is best? Will screwing from back be sufficient or should I screw from front and fill screw holes?
The ladder is already installed in ceiling.

In the last glue review I read, Titebond III came in first for every possible application - interior or exterior.
Having said that, a chunk of 1/2" MDF attic door sized will still be pretty heavy and somewhat dangerous if not fastened completely. I would prefer instead to use some thing of a construction adhesive type product plus the screws. Fully cured, it would have a ton more tooth and grab than thin yellow wood-type glues.

Yellow glue, run a couple of sheetrock screws either way, whichever is more convenient, remove the screws the next day, patch & paint.
