How many lives need to be be lost, and in what time frame, how many injuries (dibilatating or not) have to take place, how much monetery loss has to occur before something should be done?
No specific reason for the deaths/injury, but when should action be taken and does it matter how many people will be effected by that acton (not those killed or injured)?

*It depends who is doing the assessment.If it's an accountant, then the point at which you take action is when the cost to the company of the deaths/injuries becomes higher than the cost of putting safety measures in place.The policy is called 'Affordable risk'----- and No, it doesn't matter how many die, it's all determined by the bottom line.
*I don't think it's important to change laws/policies to reduce injuries/deaths until someone rich or popular gets hurt or dies. Or someone that's white and real cute. (Like the Ramsey girl)..(No, I don't really believe that - I was being cynical)
